H2H-Homeless 2 Housing

Our H2H program is MCMM’s initiative to provide full-circle case management and follow-up services to individuals that were formally homeless or that our currently homeless.  MCMM provides support in a variety of ways by providing an opportunity for employment and volunteering while assisting these individuals in all aspects of navigating their path to stability.  MCMM provides assistance in housing, document restoration, legal assistance as well as help facilitate mental health services, substance abuse treatment and after-care support.  Our H2H staff is our “Peer Support” team which has become a vital part of MCMM’s Street Outreach and Transitional Support teams.
